Barrie Copeland, 79, from Darlington, was a passenger on board the Marella Discovery (operated by TUI) on August 23, 2018.

While in the broadway show lounge of the ship looking for a seat, Mr Copeland fell down steps and suffered a head injury which tragically led to his death on September 21, 2018.

Mr Copeland's inquest closed on April 27, 2020 and the conclusion was recorded as 'Accident'.

Now, TUI has been issued with a regulation 28 report, aimed at preventing future deaths in similar circumstances

James E. Thompson, assistant coroner for County Durham and Darlington, wrote: "During the course of the evidence heard at the inquest, a witness described the steps Mr Copeland were covered in carpet so as to make them difficult to recognise as steps, and [it was] difficult to differentiate the change in floor level at the venue.

"The lighting in the lounge can be altered and was not fully lit at the time of the accident.

"The steps could therefore be the cause of future accidents. This is of particular relevance to the infirm and those persons with poor eyesight.

"The inquest did not have any evidence to indicate whether the scene of the accident had been examined by TUI.

"In my opinion, action should be taken to prevent future deaths and I believe your organisation has the power to take such action."

Supporting the conclusion of 'Accident', a medical cause of death was also given as 1a. aspiration pneumonia, 1b. acute intracerebral haemorrhage, 1c. accidental trauma while on anticoagulation for atrial fibillation, and 2. ischemic heart disease & atrial fibillation.

TUI has until June 25 to respond to the report.

A spokesman for TUI said: "We were deeply saddened to hear about Mr Copeland. We can confirm we will be responding to the Coroner in due course.

"As this is an ongoing investigation, it would be inappropriate to comment further.

"We would like to reassure customers that we regularly audit our ships in respect of health and safety."
